This waiver form will be used at all SQT's and the Summer State Tournament. Players will have to fill this out before they can compete. We recommend having all your players fill one out at the beginning of the Spring and scan them. Then, you can email them to the host and to the Summer State Tournament director, and not have to get multiple ones filled-out by the players
